Required Health Registration Information

Print the required documents, have your child’s health care providers complete and sign it, then return it to the school nurse. Documents can be emailed to [email protected]. 
There is a 20- day grace period to provide documentation of shot records or your child will be withdrawn from school.

Lead Testing Certificate         Medication Administration in Schools

Call Mrs. Seidler if your child will be absent due to illness or injury.
Please do not send a sick child to school.
Notify Mrs. Seidler as soon as possible of any illness due to Covid-19, strep throat, flu, or chicken pox.
Please notify the office if your phone number changes.
